What Employers Must Know: Maid Insurance & Health Rules

When hiring a domestic helper, it only makes sense that you should take care of their healthcare needs. To ensure employers do not shirk this responsibility, the Ministry of Manpower (MOM) has laid down mandatory requirements, including purchasing health insurance for your maid.
Additionally, employers are required to bring their maids for regular health checkups under Six-monthly Medical Examination (6ME) rules. These checkups are relatively simple, and can be performed at any number of outpatient clinics in Singapore.

Cost of Hospitalisation for Domestic Helpers

As an employer, you are fully responsible for your helper’s medical care and hospital bills if she becomes sick or injured. Foreign domestic helpers are not eligible for subsidised healthcare in Singapore. This means their medical costs are often higher than you might expect. Additionally, employers are required to bring their maids for regular health checkups under Six-monthly Medical Examination (6ME) rules. These checkups are relatively simple, and can be performed at any number of outpatient clinics in Singapore.
Here’s a breakdown of potential hospitalisation costs:

Treatment / Ward Type Estimated Cost
Stay in Ward C From S$264 per day
High Dependency Ward From S$770 per day
Intensive Care Unit From S$1,152 per day
Day Surgery (Facility Fee) From S$196
Surgeon Fees S$400 to S$10,000
Anaesthetist Fees S$500 to S$1,900
Inpatient Attendance Fees S$210 to S$630 per day
Note: Figures above are a guide and may vary by hospital, medical condition, and treatment needed.

How Maid Insurance Can Help

As you can see, the cost of hospitalisation is nothing to be scoffed at. Thankfully, employers are required to purchase a maid insurance plan for their domestic helpers.
At a minimum, MOM requires that maid insurance plans provide the following:
🩺
At least S$60,000/year for hospitalisation & surgery
75% co-payment by insurer for claims above S$15,000
🛡️
At least S$60,000/year for personal accident
Paid out for death or total & permanent disablement
Why it's a good idea to go above and beyond
Basic coverage is required, but opting for more can help you avoid unexpected costs and better protect your helper.
🏢
Employers are liable for 25% co-payment
Insurers only cover 100% of medical bills up to S$15,000. Above this, employers pay 25%. For example, a S$30,000 bill means you pay S$3,750.

Some plans (like HLAS Maid Protect360 PRO) waive this co-payment if your helper requires hospitalization or surgery (Enhanced Plan and up).
💰
Basic maid insurance only covers hospitalisation costs
Hospitalisation coverage helps with treatment costs, but not with recovery needs. If your helper can’t work after treatment, you’ll need to send her home.

Some plans (like HLAS Maid Protect360 PRO) offer add-ons for critical illness benefits, temporary alternate maids, and free tele-consultations.

How to Choose the Right 6ME Clinic and Maid Insurance Plan

6ME Clinics: Choose a clinic near your home so your helper can get to her appointments safely and quickly. Consider how busy the clinic is—shorter waiting times mean less disruption to her day. Pricing for 6ME packages is usually similar, so focus on convenience and accessibility.

Maid Insurance: Don’t just pick the plan with the lowest premium. Compare benefits and consider what’s most relevant for your needs—especially coverage for co-payments, repatriation, and temporary help if your maid is unable to work.

📝
Key things to consider for your maid insurance plan:
  • Can you afford the 25% co-payment above S$15,000 ? Some plans offer a waiver of co-payment.
  • Does the plan cover termination and repatriation costs if your helper needs to return home?
  • Is there temporary maid replacement coverage while your helper is unable to work?
  • Will you need to pay the S$5,000 security bond? Look for a plan that offers a Letter of Guarantee.

Understanding 6ME for Domestic Helpers

6ME stands for Six-monthly Medical Examination, an initiative by Singapore’s Ministry of Manpower (MOM) to regularly monitor the health of foreign domestic helpers.

It’s a simple checkup performed at approved outpatient clinics in Singapore, but employers must follow strict requirements:

🏢
Key 6ME Requirements
  • Performed once every six months
  • Conducted only by a Singapore-registered doctor
  • Paid for by the employer
  • All test results are submitted directly to MOM by the clinic
  • Employers/representatives cannot be present during the check-up

What Does 6ME Check For?

6ME appointments cover mandatory tests as required by MOM. Before each appointment, MOM will notify you which tests your helper must complete.

Health Screening Item Frequency
Pregnancy Every 6 months
Venereal Disease Research Laboratory (VDRL) test
(Screens for syphilis and related conditions)
Every 6 months
HIV Every 2 years
Tuberculosis (TB) Once, after 2 years in Singapore
Checks on BMI and visible signs of abuse Every visit

Measles Vaccinations

  • If your household has a child under 7 years old who has not been vaccinated for measles, you must ensure your helper is immune, vaccinated, or has a vaccination appointment booked before applying for or renewing her work permit.
  • If your helper is not immune or fully vaccinated, she will need the MMR (measles, mumps, and rubella) two-dose vaccine. This can be done during her 6ME check-up.
